A 90-year-old male is experiencing a painful ulcerative lesion on the right lateral border of the tongue. The lesion has remained unchanged for 3 weeks. An ENT specialist prescribed antibiotics with no improvement. During the clinical examination the clinical characteristics of the lesion such as elevated and indurated margins, along with its prolonged duration raised concerns regarding the possibility of squamous cell carcinoma. An incisional biopsy was performed, and the histopathological report indicated a granulomatous morphology rich in lymphocytes and eosinophils. The final diagnosis is Traumatic Ulcerative Granuloma with Stromal Eosinophilia (TUGSE). The exact cause of the pathogenesis remains unclear, although trauma seems to play a role in the onset of TUGSE. In our case, we hypothesized that the trauma might be linked to sharp teeth. We ruled out this possibility by smoothing the sharp edges of the teeth. The lesion underwent spontaneous healing within 10 days after the biopsy.

The first condition is known as "hairy tongue," which is a benign condition that involves elongation and discoloration of the filiform papillae on the dorsal surface of the tongue.
On the other hand, the second condition, known as " proliferative verrucous leukoplakia," is considered a potentially malignant disorder. It is characterized by its chronic persistent, multifocal lesions, resistance to treatment, and a notable risk of malignant transformation.

Angina Bullosa Hemorrhagica (ABH) is characterized by recurrent, easily ruptured hemorrhagic blisters. Can occur at any location of the oral cavity, particularly in sites exposed to trauma.
